New Patients

  • Please before you schedule appointment call your insurance and ask if Town Family Doctor is in their network.
  • Please read our Privacy Policy prior to filling out the forms.
  • Please print out and fill in the following forms prior to your visit. If you are not able to print these forms out, don’t worry, they will be provided to you at the office.
  • If needed, please arrive 20 minutes prior to your scheduled visit time to allow yourself enough time to fill out these forms.
  • If your insurance is not on our list, please call the customer service phone number located on the back of your insurance card, to verify the participation with that specific plan.
  • Please bring your insurance card and driver’s license with you.
  • All co-pays are due on the day of the visit. You may be asked to pay any deductible or outstanding balances on your account prior to being seen. We sign contracts with insurance companies and we must abide by their policies on the collection of co-pays at each visit.
  • There is a $30 fee for not showing to an appointment or canceling on the day of appointment. This fee can be waved in case of emergency. By not letting  us know in time that you will not show up for an appointment, you are taking an opportunity from other patient to be seen.
  • At your first visit make sure to ask about the Patient Portal. You will use this Portal to view your appointments and results, communicate with your provider, request a refill, and update your information.
  • Self-pay patients and walk-ins are also welcome.
  • If you are secondary on your insurance, please have the Social Security Number and Date of Birth of the person who is primary on that insurance.
  • If you have primary and secondary insurance, please bring both. The first visit usually takes approximately one hour, so please allow enough time for the visit.
  • For patients with Medicare as your primary insurance, a general physical is allowed only on the first healthcare visit within six months of getting Medicare (this is also known as the Welcome to Medicare Physical).
  • Workers’ compensation, auto, and health insurance cannot be used interchangeably for the same health problem.
  • List of accepted insurances: Anthem, United, Aetna, Cigna, Humana, Medicare, UMR, Coventry, Unicare, Humana Care Source, Wellcare, and many more (Every insurance has plans that have limited participation. That means that  even we are in some plans  with a particular insurance we might not be  allowed to participate in all of them. Please contact your insurance and confirm that we are in your plan. 

Please note: Regardless of your insurance coverage, you as a patient are responsible for making sure your medical bill is paid.
