Please read our Privacy Policy prior to filling out the forms. Print out and fill in the following forms prior to your visit. If you are not able to print these forms out, don’t worry, they will be provided to you at the office. Please arrive 20 minutes prior to your scheduled visit time to allow yourself enough time to fill out these forms. We accept the following insurances: ANTHEM, UNITED, AETNA, CIGNA, HUMANA, MEDICARE, UMR, COVENTRY, and UNICARE. For all others, please call the customer service phone number located on the back of your insurance card to verify the participation with that specific plan. Please bring your insurance card and driver’s license with you. Self pay patients are also welcome. If you are secondary on your insurance, please have the Social Security Number and Date of Birth of the person who is the primary on that insurance. The first visit usually takes approximately one hour, so please allow enough time for the visit.
Generally preventative visits (physicals) that are designed for screening diseases cannot be done at the same time as problem-oriented visits (for insurance reasons). For patients with Medicare as your primary insurance, a general physical is allowed only on the first healthcare visit within six months of getting Medicare (this is also known as the Welcome to Medicare Physical).
Worker comp Auto and Health insurance cannot be used interchangeably for the same health problem.
All co-pays are due on the day of the visit. You may be asked to pay any deductible or outstanding balances on your account prior to being seen. We sign contracts with insurance companies and we must abide by their policies on the collection of co-pays at each visit. Please note: Regardless of your insurance coverage, you as a patient are responsible for making sure your medical bill is paid.
